 To obtain your passcode, follow these simple steps:
 
 Fill out the form below with your email address. 
 Submit the form and wait for confirmation. 
 Receive your passcode via email. 
 
 This passcode is unique to each parent/guardian. Please do not share it 
with your spouse or another guardian. If another code is needed, please
